Menopause Supplements: What the Evidence Actually Shows

Walk into any pharmacy or scroll any wellness site and you'll find dozens of supplements promising to fix hot flashes, mood, sleep, or "hormone balance." Most of them get marketed with the same confidence, regardless of how much research actually backs them up. That's not how we do it here. Some of what follows has real evidence behind it. Some is genuinely promising but early. Some is popular despite thin support. We think you deserve to know the difference before you spend money or make a decision about your body.
How we're grading this
Before recommending or dismissing anything, we ask the same four questions about every ingredient:
- Is there actual human research — not just a mechanism that sounds plausible, or evidence from animal studies?
- Does the dose studied match what's typically used — a promising result at 3,500mg doesn't tell you much about a product containing 200mg?
- Was the research done in menopausal women specifically, or is it being borrowed from research on stress, athletes, or an unrelated population?
- Do independent studies agree, or does the evidence conflict — and if it conflicts, why?
We sort everything into four categories: strong (consistent human evidence, ideally in the relevant population), emerging (real but limited or early research), mixed (studies disagree with each other), and weak/theoretical(biologically plausible, but not well-demonstrated in people). If the evidence is weak, we say so — including for ingredients in our own product.
Strong evidence
Vitamin D. Well-established for bone, immune, and muscle health, and deficiency is common enough in adult women that correcting it is genuinely worthwhile — not a menopause-specific finding, but a real one.
Vitamin B12 and folate (as active forms). Well-established for energy metabolism and nervous system function, particularly relevant for women with absorption issues or common genetic variants affecting folate metabolism.
Magnesium. Reasonably strong evidence for sleep quality and general relaxation — one of the more consistently useful nutrients for menopause-adjacent sleep complaints, though it's not a fix for every sleep issue.
Black cohosh, with real caveats. This is one of the most-studied botanicals for hot flashes, and it's genuinely a mixed bag depending on which review you read. Individual randomized trials have shown real benefit for hot flash frequency and severity, particularly in early menopause. But a 2012 Cochrane systematic review — generally considered the gold standard for evidence synthesis — concluded there wasn't enough consistent evidence to confidently recommend it, largely because trials used different extracts, doses, and standardizations, making it hard to compare results directly. Our honest read: promising and worth discussing with a provider, but not a guaranteed fix, and standardization of the specific extract matters more than the marketing usually lets on.
Emerging evidence
Ashwagandha (including KSM-66). Well-studied for general stress, cortisol, and sleep in adult populations, including multiple randomized controlled trials. Menopause-specific research is thinner — a small 2024 pilot study found improvements in menopause-specific quality-of-life scores, but larger, menopause-focused trials are still needed before it's fair to call this "proven for menopause" specifically, as opposed to "proven for stress, with early signs it may help here too."
Rhodiola rosea. Proposed as a possible natural selective estrogen receptor modulator, with research suggesting benefits for energy, mood, cognitive function, and memory relevant to the menopause transition. One notable placebo-controlled trial found it significantly more effective in combination with black cohosh than either alone — worth knowing if you're evaluating a product containing rhodiola by itself, since the strongest evidence involves a combination.
Genistein (a soy-derived isoflavone). Genuinely promising, specifically for hot flashes — research has linked it to reduced frequency of vasomotor symptoms, along with some evidence for mood-related benefits. Important context: genistein is a phytoestrogen, meaning it has real estrogenic activity in the body, which is part of the proposed mechanism. That's worth knowing plainly, not glossing over.
Soy isoflavones and red clover more broadly. Similar story to genistein — evidence leans positive for hot flashes specifically, less convincing for other symptoms like sleep or mood, and results vary depending on the specific isoflavone profile and dose used.
Diindolylmethane (DIM). Has real, documented effects on how the body metabolizes estrogen — but here's the important part: a 2025 study published in the Menopause Society's own journal found that DIM measurably altered estrogen metabolite profiles in postmenopausal women concurrently using transdermal hormone therapy. If you're using hormone therapy, this is genuinely worth discussing with your prescribing provider before adding a DIM-containing supplement — this isn't a minor caveat, it's a real interaction worth knowing about upfront.
Calcium D-glucarate. Supports a liver detoxification pathway (glucuronidation) in principle, and is commonly paired with DIM. Direct human evidence on symptom outcomes — as opposed to the underlying mechanism — is still limited.
Maca root. A randomized controlled trial found benefit for sexual function at a 3.5g daily dose — worth knowing if a product you're considering uses a meaningfully smaller amount, since the studied benefit doesn't necessarily scale down proportionally.
Curcumin and CoQ10. Both reasonably well-studied for general anti-inflammatory and cellular-energy support, respectively — strong ingredients in general, though menopause-specific outcome research (as opposed to general health research) is still limited for both.
Mixed or limited evidence
Evening primrose oil. Genuinely mixed. Some trials show modest benefit for hot flash severity, particularly short-term; a head-to-head comparison found it less effective than black cohosh, and a broader review found no significant difference from placebo for hot flash frequency. Reasonable to try if you're curious, but we wouldn't oversell it.
Chromium. Commonly included in multivitamins for metabolic support, but evidence for blood sugar-related claims specifically is inconsistent across studies. We'd stop short of implying it meaningfully affects blood sugar "control."
What we won't do
We won't tell you a supplement "balances your hormones," "reverses menopause," or "replaces hormone therapy" — because none of what's above does any of that, and claiming otherwise would be the same overselling we're trying to help you see through everywhere else. We also won't pretend the weaker-evidence ingredients on this list are stronger than they are just because they're popular, or because a product (including our own) contains them.
Where this leaves you
None of this is a substitute for medical evaluation, especially if your symptoms are significant. What it is: a starting point for an informed conversation, whether that's with your doctor, or just with yourself, about what's actually likely to help versus what's mostly marketing. Foundational nutrition — correcting an actual deficiency, supporting the systems most affected by hormone change — is a genuinely reasonable thing to prioritize daily. Expecting any single ingredient to resolve a specific, significant symptom on its own usually isn't.

Do menopause supplements actually work? It depends entirely on the specific ingredient, not "supplements" as one category.
What is the best supplement for hot flashes? Black cohosh and soy-derived isoflavones like genistein have the most direct evidence for hot flashes specifically, though results vary between individuals and neither works for everyone.
Is black cohosh safe and effective for menopause? Individual trials show real benefit for some women, particularly in early menopause, but a major systematic review found the overall evidence inconsistent enough to stop short of a confident recommendation. Reasonable to discuss with a provider; not a guaranteed fix.
Can I take DIM if I'm on hormone therapy? Talk to your prescribing provider first. Research has found DIM can measurably alter estrogen metabolism in women using transdermal hormone therapy — a real interaction, not just a theoretical one.
